On October 2, 2014, Antonio Mendoza placed his bet at a PCSO outlet in Barangay Dacanlao in Calaca, Batangas.

He was supposed to win the jackpot prize worth P12,391,600 after hitting the winning combination 09-21-31-36-40-41 in the Lotto 6/42 game.

However, one of his grandchildren crumpled the ticket. Another grandchild tried to smoothen the ticket by ironing it out, but extreme heat from the iron burnt the ticket made of thermal paper.

Mendoza presented the damaged ticket to the Philippine Charity Sweepstakes Office hoping that he can still claim the jackpot. Unfortunately, it was no longer machine-readable.

The supposed to be instant millionaire went home empty-handed.
